Hospital2Home Care Transitions: A free 30-90 day program for adults (18+ including those living with intellectual and developmental delays) living alone or with caregivers that have recently had a hospitalization related to Dementia, chronic medical conditions, or covid-19 complications (including pandemic related isolation). The goal is to reduce hospital recidivism rates and the need to stay in acute care by assisting adults with memory loss and other vulnerabilities, and their caregivers with resources to meet his/her health and social needs while also providing respite and education.
